Yes, you need to change the stud.

The orientation of the outlet is pretty irrelevant to be honest. You can even orientate the head so that a standard BR9ECM spark plug fits (just) within the standard bodywork if you want. It will have no bearing on performance.

The "correct" orientation is however, with the hose stub pointing down.
