Holy thread bump Batman!!

My 28s dry clutch is slipping badly now. Been doing it a bit now and again but it has got worse in the last few rides.

What bits do I need? What Matt said above?

A list would help as I don't have a clue what needs replacing, obviously I will get part numbers etc.
