The center bolt of the puller has a flat surface to mate with the crank end.  I did not use any type of adapter.  I'm not too concerned about the crank end because I have a new crank to install.  
I used the puller exactly as you described.  I applied very light oil to the 27mm male threads on the outer part of the puller and made sure it was screwed all the way in.  Then I used my 18-inch breaker bar to apply torque.  The flywheel would not budge.  I thought I had the flywheel loose but realized the outer part of the puller  was pulling out of female threads of the flywheel.  The male threads on the puller are stripped.  I can't really see the female threads of the flywheel very well and I hope they are not stripped as well.  I'm going to try a new puller and see what happens.
